"failed to intialize" = this error can popup if you have /noexecute=optin/
in the boot.ini file,open/read the boot.ini file with NotePad and see if the call 
is in path statement if so remove it and then try it again. (reboot after the edit)
Take care with the boot.ini file it boots the machine.
If you are unsure make a backup copy b/4 you edit.
i.e. save as boot.in_ then edit boot.ini file.

Note :the boot.ini is a hidden system file and is taged as read only.
You must enable show all files and folders to to view it and you must remove the 
read only tag to edit the file.
