I had a very similar problem and discovered that, as suggested previously in this
thread, it is all down to the detection of TV Outputs. If you go into the Tools
tab of the Nvidia display sattings for your graphics card and uncheck the "Force
TV Detection" tab then the ghost monitors will stop appearing in Hardware monitor.
For the uninformed I am referring to specific display settings and not the dsesktop
ones. In other words you can access this by right clicking on the desktop and left
clicking 'Nvidia Display'. I hope this helps!
