You need a Boot Disk with Format.com on it. 
I don't think Format.com is on the boot disk that you download, its in the Windows\Command 
folder. You may as well put Fdisk.exe on there as well. If you HD is greater them 
64Mb then you will need a newer version of Fdisk from Microsoft.

Set your boot sequence in the BIOS so it boots from the floppy drive first. Insert 
the boot disk and startup or restart your computer. At the A:\ just type Format 
C: or what ever drive letter you want to format, follow the instructions. If 
you want to copy the system files at the same time then use Format C: /s
