Question about 'Setting up Dial-Up Networking'

I have a question about my ISP connection. I have downloaded the most current driver
for my modem (56K US Robotics). I have TRIPLE checked my ISP phone number, settings,
etc. My modem dials out, I hear it communicating with the ISP server, and then,
just as I am almost connected to the ISP, it abruptly clicks, and reports a failure
to communicate. The message also says "Check your login and password". Again, I
have checked this over and over, and have made sure I used the proper case type.
