open regedit - go to HKEY_CLASSES_ROOT\folder\shell create a new key with the program name under that, create a key named command with a string value of the path to the exe.

For secure versions of Windows (such as XP), you can go to HKEY_CURRENT_USER\Software\Classes, create a key named Folder, create a key named Shell, create a key with the program name, create a key named Command, and then change the default value to be the path to the EXE. If any of the keys I told you to create already exist (not very likely), you can go ahead and use the existing ones.
