misaligned mouse

Does anyone have any tips on what to update/patch for a misaligned mouse pointer.
Sometimes it is right on sometimes it is off and I have to hold it up and usually
left of what ever I am trying to click.
If I minimize then maximize and move the mouse around a bit, I can get it aligned
for awhile, but it never lasts.
