Can't start Task Manager, no way to kill process

When Vista gets locked up by a rogue process, I would normally start the Task Manager 
to kill the process.  Incredibly, this often isn't possible: Ctrl-Alt-Del does nothing, 
and Start Menu/Run.../taskmgr also does nothing.

How do I kill a process if Ctrl-Alt-Del doesn't work, and there's no way to launch 
the Task Manager?  This makes no sense -- I have to reboot the whole machine every 
time Firefox hits a plug-in it can't cope with?
