The open screen hazes over to opaque whitish colour accompanied with non-operational 
keys and mouse, then the screen goes black, the mouse cursor is visible and movable 
but I cannot right click and get any anything to happen. The keyboard does not produce
anything except an alt-ctrl-del which occasionally, but not always, brings up a dialog 
box…

Titled:  Login process has failed to create the security options dialog
with a dialog message of:  
Failure - Security Options

Only steps I can get to work to proceed:
Shut down (hard/improper via OFF button) and then restart in SAFE mode; wait to load 
software completely, and then restart in normal mode. It’s not consistent.  Google 
search with key words from error message show this is not just a Toshiba issue. 
It crashed today, but there is no record in the problems log at all.

Anyone got any ideas?  thanks, Jim

This is not the final answer! Try something for me. While you are in that blank screen with a cursor, press your Ctrl/Alt/Delete to see if you can get to your Task manager. If so click on file/new task (run) and enter "explorer.exe" (without the quoats) in the slot provided and then hit Enter. If this works and gets you a desk top we will look into your registry next.

Only a long wait produces the error message, a black screen, or a desktop background with no icons and no reaction from any keys. Only thing that works is a hard boot. today as part of diagnostics, I loaded SP#1 for Vista, and will track to see if crashes continue. Jim

Let me know, If that doesn't fix the problem you may be forced to fix your bootloader.

Yesterday I loaded Vista Service |Pack 1, I'm monitoring for crashes but so far it's 
been OK.  If that doesn't' work though, it's back to reloading as you suggest.  Jim

SP1 replaces a lot and re-writes a lot. If that doesn't fix things let me know. Be aware that there are some hardware devices and software that don't work with SP1. Good Luck!

I am running Vista Home Premium, and I seemed to start getting these crashes after SP1.

My "family safety" dialog prompts also seems to have been suppressed since SP1. I will try disabling that feature.

I have started to see this behaviour too directly after the latest Windows update was applied last night. I haven't had problems before this with SP1. I am going to do a system restore to before the update and see if that helps

Be aware that Sys Restore may not be a viable answer. The more major updates seem to del. restore points, Presumably to prevent corruption after the update.

Yes, you're right - this didn't work at all. The computer came with a security suite called Bullguard which I uninstalled in safe mode. When I rebooted everything was back to normal. I've installed Trend Micro Internet Security instead and am keeping an eye on things. Of course, Windows update now wants to install the latest updates ....

Thanks for the update. I am now researching BULLGUARD. Didi you install this or did it just mysteriously show up? AV's have caused more issues with Vista!!!!!!!!!!

Bullguard was pre-installed by the computer vendor. I thought the best place to start with was the security software. It seems to me that there are 2 schools of thought on the causes of this problem - either security software or hard disk issues. Hopefully it's the former in my case.

Good luck. There has been a lost of issues on the security software thing. I don't points fingers at to who caused what, just try to generalize as much as I can.

I'm getting these exact same symtoms. Running vista home premium 32 bit on an acer laptop. Security software is bitdefender. I get the error frequently, in particular recovering from sleep mode (so I disabled). Most frequently suggested cause on the net is a hard disk problem (particularly S.M.A.R.T data errors). I ran some basic diagnostics but couldn't find a problem. I'd be very interested if it's a HDD problem or some other cause. A lot of people seem to have the same problem.

I'm going to dump a lot of stuff on you really quick. The first link is the easieast to comprehend. It get more technical for there on. It is just too much to type out on this forum. The first link shoud fix 95% of the boot failures. If you can boot I would suggest you get "Vista Boot Pro". I do not usually promote stuff but this is good and very easy to use for most problems. Most of the links I am providing are the exact same information and I only do so as possible some people may understand one explanation where another makes no sense at all. My new HP laptop has this issue. I uninstalled NIS 2008, and that seemed to reduce the number of occurances, but it did not eliminate the lock ups completely. Some of the other forums also hinted at an HD issue with Western Digital, so I checked my specs, and sure enough, I had a WD HD. I contacted HP suport and spoke to an Indian gentleman named Nelson Montana. They instructed me to run a diagnostic on the HD, and it came back saying that I should replace the HD. Luckily, my machine is still under warranty, and they are sending me another HD. I'm keeping my fingers crossed.
