one frequent cause of this is opening a file directly from a floppy (a very poor
practice). This puts the file on the Recent Files list, so Explorer periodically
"looks" for it. Check there, and remove any file references to the floppy. Sometimes
people rather stupidly end up with a reference to a file on the A:\ drive in their
startup folder.
