Windows XP crashing randomly, then blue screen

At least once a day my computer crashes. It sometimes happens when I am listening to music in Media Player. I can't play Sims 2 because the game crashes. Even when just trying to open My Pictures, I get "send error report" I get this same message when using iTunes, in Media Player, and in accessing My Documents, My Music.. I have replaced the power supply, changed memory, formatted my whole computer and re-installed Windows. I am not sure what should be my next step in finding the problem & solution. Main issues are receiving blue screen first with error code then system completely crashing and receiving constant "send error reports" from Microsoft
