I had the same problem and the only way I got it resolved was by deleting the destination file (what the no longer existing shortcut points to) and execute the custom key combination. WindowsXP begins to search for the file and lets you browse for it too. At this point I hit cancel and do the key combination again. After 3 attempts, Windows tells you the file no longer exists and asks if you want to delete the shortcut. It worked for me. If you want to reuse the same key combination for something else, you will need to reboot before Windows lets you set it for another shortcut.
