You have been very systematic in your approach, so the problem lies in the Download
folder. I would suggest that you go to that folder, right click on it, select properties
and check whether there is anything unusual displayed.
If you cannot find anything, copy the folder and see if the copy displays the same
problems.
Finally, check for spyware using the guidelines as set out here
