After 6 months of searching for a difficult solution to a wicked simple problem,
I've finally found my missing icon (and I feel like an idiot for not finding it sooner).
For what it's worth, I'll post my solution here in case it helps anyone else:
My missing icon was the Dell Wireless WLAN Utility icon. Apparently, in a drunken
stuper, I had unchecked the box next to "WLTRAY" in the startup tab in the system
configuration utility (i.e. msconfig).
I continue to have the problem with the incorrect icons in the past items section
of the Customize Notifications window, but I'm going to try to let that go...maybe.
On another note, imagine all the company-paid time and resources this post has enveloped
over the years. Thank God for down-time and fringe benefits.
