Without reproducing it myself now, I can't be certain exactly what it is (a checkbox?) but I have seen it dozens of times and never had it stick in a loop I couldn't get out of, so it must be easy. You also have two other ways of disabling the Narrator startup in the registry which I know that you know about - Regedit directly (delete the value in the Run key) or Autoruns, Logon tab, avoiding msconfig completely.
