Right click crashes XP

I've got a huge problem that's only existed for a few days.
I honestly do not know what is causing it - my system does not change a lot.
Anyway, whenever I right click any shortcut (while in windows), my system locks up.
The cpu for explore.exe jumps to 99% and stays there.
I have to open the task manager, kill explore.exe, then either restart or log out.
I can double-click shortcuts, no problem.
I can make new shortcuts, and move them around.
It only happens when right clicking shortcuts.
This is what I've tried so far:
1) Uninstall/reinstall SP3
2) Used ShellExView to turn off every context menu handler.
3) Cleaned the registry, stray files, etc.
4) Uninstalled some old apps
I'm going nuts here - if I forget for a moment and right-click a shortcut, or drag
something over the top of a shortcut, I'm screwed.
Can anyone offer any help?
